Key activities:

  • Implementation of post fracture screening at fracture clinics for fragility fractures.
  • Self-management programs to provide those newly diagnosed with appropriate information and tools to cope with osteoporosis and become an active participant in their care.

  • Fracture Clinic Osteoporosis Screening Program.

    Screening Coordinators have been deployed at select high and medium volume fracture clinics across Ontario to screen and identify patients who might have suffered a fragility fracture (defined as a fracture sustained from a fall from standing height or less). These patients need appropriate treatment for their fracture as well as assessment and treatment for the underlying reason for their fracture, osteoporosis. The Coordinators assess and educate at-risk patients about their risk of osteoporosis, facilitate access to diagnosis, treatment and management.

Long-term outcomes:

Ensure integrated fracture and rehabilitation services across Ontario, resulting in a reduced number of re-fractures in the province.

Fracture Fighters: The Ontario Osteoporosis Strategy for Inpatient Rehabilitation

Key activities:

The primary objective of Fracture Fighters is to integrate osteoporosis management into existing inpatient rehabilitation services provided to patients post-fracture, thereby preventing future fractures. 

Key activities include: 

  • An environmental scan of Ontario inpatient rehabilitation facilities 
  • The development of osteoporosis guidelines and protocols; health care professional and patient resource materials; strategies for implementation of osteoporosis best practices; and a Fracture Fighters website
  • Training of site-based Fracture Fighters clinical coaches through provider-training sessions and ongoing support 
  • The formation of a clinical coach advisory group 
  • Patient follow-up survey to assess the impact of the strategy on patient behaviours and outcomes

Ontario Osteoporosis Strategy for Long-Term Care

Key activities:

  • Work with the Long-Term Care Sector to promote and implement evidence-based osteoporosis and fracture prevention care strategies

  • Educate long term care staff and service providers regarding the appropriate management of osteoporosis and prevention of fractures

  • Promote greater awareness and evidence-informed care decisions regarding osteoporosis, falls and fracture prevention

  • Develop user-friendly and appropriate resources for long term care staff and service providers

  • Advance the long term care-focused osteoporosis, fracture prevention and falls knowledge base through research, literature reviews, surveys, publishing, presentations and collaborative partnerships.

  • Develop guidelines and algorithms for falls prevention programs in long term care facilities.

  • Educate staff about the importance of falls prevention and effective strategies

  • Promote the use of protective devices such as hip protectors

  • Encourage appropriate supplementation with calcium and vitamin D for residents of long-term care facilities.
